0978 I Cannot Stay Still - A Case of Sleep-Related Rhythmic Movement Disorder

Abstract Introduction Sleep-related rhythmic movement disorder (SRRMD) is characterized by repetitive motor behaviors involving large muscle groups that occur predominantly during stage N1 sleep. SSRMs are common in infants and children but they usually spontaneously resolve before adolescence. This case presents a rare adult of SRRMD. Report case(s) A 60-year-old man initially to clinic for evaluation movements needed initiate Patient reports the problem starting his childhood, where he needs rock roll intentionally bed fall asleep. He had been prescribed clonazepam 2mg taking this along with doxylamine melatonin nightly, still frequent nocturnal awakenings due body rolling. specifically denies any hypnic kicking/jerking, or discomfort legs while falling has no excessive alcohol consumption, marijuana, illicit drug use. Differentials patient’s condition include hyperarousal, chronic anxiety, iron deficiency, sleep-related breathing disorder, disorder. consequently underwent an in-lab diagnostic polysomnography study which showed evidence disorders AHI 1.6 (AASM 3%). Sleep efficiency was low at 60.8% sleep architecture decreased REM N3 Rhythmic were noted it manifested transient activity both limb & chin EMG leads also whole rocking video recording. Moderate periodic PLM index 17. seen 6 months later persistent symptoms dose increased slowly 2.5 mg then 3 mg. On clonazepam, patient resolution symptoms. Conclusion Our describes who SRRMs commenced childhood persisting well into adulthood. Reported cases SRRMD few, have associated apnea, behavior RLS, many remain idiopathic such as patient.
